Community Stroke Team
Temporary Full-time

POSITION SUMMARY: Using the principles of person-centered care, the rehabilitation therapist is responsible for implementing goal focused interventions in the home and various community settings under the direction of and in collaboration with allied team members. Specific areas of program focus include cognitive and communication strategies training, independent living skills, physical functioning and facilitating community integration. The rehabilitation therapist will also provide client-specific consultation and education/training/inservices related to stroke rehabilitation for service providers, caregivers and families.

REPORTS TO:         Manager, District Stroke Program

DATE REQUIRED:  As soon as possible

SALARY:                   $31.08 to $35.79 per hour


- Undergraduate degree in health or social science and a college diploma in a related field or equivalent stroke rehabilitation experience.

- Clinical expertise and experience in stroke rehabilitation and demonstrated knowledge of current stroke rehabilitation best practices.
- Experience in both hospital and community service delivery is an asset.

- Demonstrated ability to practice effectively in an interprofessional collaborative environment.
- Sound understanding of the role of a variety of rehabilitation professionals in the provision of stroke rehabilitation.
- Demonstrated ability to work independently
- Demonstration of general skills and abilities in the areas of facilitation, teaching, collaboration, written and verbal communication and observation
- Working knowledge of and experience with theories and principles guiding adult learning and patient self-management approaches.
- Excellent communication, interpersonal, organizational, time management, problem-solving and innovation skills.
- Proven flexibility and a high level of initiative.
- Evidence of continued professional growth and/or professional development
- Appreciation of resource utilization
- Computer skills, particularly Meditech, MS Office, and/or other software
- A valid Ontario driver’s license
- Current Basic Cardiac Life Support certification
